Your card says PPO, or your plan lets you choose any dentist and pay a share of the fee. Signature Smiles accepts most PPO dental plans, files the claim for you and settles directly with the carrier. You pay only your portion, and you know that number before treatment starts.

DHMO — we cannot

Please check first

Your card says DHMO, DMO, managed care, or your plan assigns you to one named dental office. Those plans only pay the office they assigned, so we are not able to bill them. Call us anyway if you are unsure — we will read the card with you rather than let you drive over for nothing.

Can I use any dentist with my PPO dental plan?

Yes. A PPO dental plan lets you choose your own dentist, which is the main difference between a PPO and a DHMO. Your plan may pay a different share depending on whether the dentist has a contract with your carrier, so the amount you owe can vary, but the choice of dentist is yours. Do you accept Delta Dental in Milpitas?

How do I find out what my dental insurance covers?

Give the practice your carrier name, member ID and date of birth, and we will contact the carrier and read your benefits back to you — free, before any appointment is made. You will learn your preventive, basic and major percentages, your deductible, how much is left on your annual maximum this year, and whether any waiting periods still apply. Doing this before you book is the single best way to avoid a surprise bill, and it costs nothing.

What if my carrier is not on your list?

Call anyway. The thirteen carriers listed above are the ones we bill most often, not a closed list. Many smaller plans and employer-specific policies are administered by one of those same carriers under a different name, which only shows up when we look the policy up. The check takes a few minutes and is free. If we genuinely cannot bill your plan, we will say so plainly and point you at the membership plan or payment options instead.

Do you take DHMO or managed-care dental plans?

No. DHMO, DMO and managed-care dental plans assign you to one specific dental office and pay only that office, so we are not able to bill them. Your card will usually say DHMO or DMO, or your plan will have named a dentist for you when you enrolled. If you are not sure which type you hold, call (408) 946-0777 and read us what is on the card — it takes a minute and saves you a wasted trip. Many employers offer a PPO option at open enrollment alongside the DHMO.

How much is a cleaning and exam with insurance?

Most PPO dental plans cover preventive visits — the exam, X-rays and a routine cleaning — at or near 100 percent, often without applying the deductible, usually twice a year. That means many patients pay nothing at all for a checkup. If your plan covers less, or if a deeper cleaning is needed because of gum disease, you will be told the cost in writing before anything is done. New patients without insurance get a free consultation, and the membership plan covers preventive visits for a flat annual fee.

Will you file the insurance claim for me?

Yes. The practice files your claim with the carrier the same day as your visit, and handles the follow-up, including any request for X-rays or clinical notes. You are not asked to pay the full fee and claim it back yourself, and you are not chasing the carrier. You pay your share at the visit and we settle the rest directly.

What happens to my benefits at the end of the year?

They reset, and anything unused is lost. Almost all dental plans run on the calendar year, so whatever is left of your annual maximum on 31 December does not carry into January. If you have treatment already diagnosed and waiting, finishing it before the year ends is usually the cheapest way to have it done. Ask the front desk what is left on your maximum — the check is free and takes a few minutes.

Can I use dental insurance for Invisalign or whitening?

Sometimes for Invisalign, almost never for whitening. Many plans include a separate lifetime orthodontic benefit that can apply to clear aligners, and it sits outside your ordinary annual dental maximum where it exists. Purely cosmetic treatment such as whitening and veneers is generally not covered by any dental plan. We will check your orthodontic benefit as part of the free verification, and both CareCredit and Cherry can spread the balance.

What should I bring to my first appointment?

Bring a photo ID and your dental insurance card to the Milpitas office. If you have a medical card from the same carrier, bring both, because dental benefits are often administered separately. A list of any medications you take is useful, and if you have had X-rays taken elsewhere in the last year we can request them so they do not need repeating.
